CPHS has a very long history of analyzing and fighting for ways to change the dollar flow to hospitals. The current funding flow program, the Indigent Care Pool (ICP), is the most current dollars to hospitals. Unlike other states, New York distributes these funds to almost all hospitals in the state. Federal rules talk about the funds being available and distributed to those hospitals that provide care to the uninsured and to large numbers of Medicaid patients. Medicaid is included because in most states, notably New York, Medicaid only pays for a percent of the cost of care. In New York it is only 70% of the costs. So ICP is meant to help those hospitals, defined as the safety net, to help them.
CPHS has actively devised substitutes and worked on definitions for those hospitals that fit a safety net definition. At one point, through a foundation grant, CPHS hired an academic consultant to develop a funding plan. This worked to an extent, but still money goes to the wrong places.
There have been two official state efforts to make changes. Commissions set up to review the system has resulted in some changes, but still money was only somewhat going to the hospitals that provided the necessary care. CPHS was a major mover in a four-year effort to get a state legislative definition for hospital requirements to be designated a safety net hospital.
